On the day that negative doubles were first invented, they were used to say "Hey, I have 4+ cards in the unbid suits!" By the afternoon of the second day, they had been changed to "Hey, I have 4+ cards in the unbid majors!" That said, if you make a negative double at the three level or higher, it's a really good idea to have (1) cards in both of the unbid suits or (2) tolerance for partner's first suit.

South could certainly have cuebid Diamonds at their first opportunity to show a limit raise or better in Hearts, but the negative double isn't wrong with such a strong holding in Diamonds. For the same reason, 3 NT looks to be a better contract than 4 , since it's likely that Hearts won't be breaking nicely. N/S take five Heart tricks, two Clubs, two Spades, and one Diamond, making four. If N/S play in 4 , they'll take the same ten tricks since they can't afford to ruff any Clubs in the South hand and still pull trump without a trump loser. West will lead the  J.

East has no reason to make a re-opening double with such strong Hearts. N/S lose one Club trick, five Hearts, and one Spade, down one. If declarer tries to pull trump, they'll lose control of the hand and go down more than one. Ugh. Best is for declarer to play high Diamonds to shorten East's trumps and then pull one round of trump somewhere in the middle of the hand. E/W lose one Spade trick and one Club, making five. It's highly unlikely that declarer will lead to their T on the first or second round of Spades--why should they? North will probably lead the T (interior honor sequence). Declarer will pitch a small Club from dummy and win in their hand with the  A. A Diamond ruff in dummy is followed by a Club to the  Q and  K. Surprisingly, the best thing that North can do is to continue with a Diamond giving declarer a ruff-sluff that they don't need! If declarer ruffs in their hand, they'll be limited to only ten tricks, so they'll ruff on the board with the  7 and pitch a small Heart from their hand. A Heart to the  A and a Heart back to dummy (covering any card that North plays) allow South to ruff, but that's the last trick for the defense.
