Once West shows the  Q and the  K, East can count 13 tricks--two Spades, five Hearts, five Diamonds (planning to ruff one in partner's hand), and one Club. If E/W try and play in 7 NT, they're going to need to be good at squeeze mechanics. Let's pretend that's the contract:

Assume that North leads the  7 (beer card). Declarer wins the  Q, plays off the  A and  K to get the bad news, and temporarily abandons Diamonds. Five rounds of Hearts are next, and South is able to pitch two Hearts, one Spade, and two Clubs--not a problem. The problem arises when declarer leads the  5 to the  A at trick nine. Here's the position:

West:  AKQ5  5 North: T82  KQ East:  6 T52  A South:  J974  J

When the  A is played, South's gives up. They'll throw a Spade, since they're staring at three winning Diamonds on the board. With the Diamonds no longer good, declarer will play a Spade to their hand and claim when North nobody has four Spades. Nicely played!
